Purpose: To discuss the design and conduct of remotely monitored exercise interventions in men with prostate cancer, including randomized controlled trials (RCTs) among men with low-risk cancer on active surveillance to RCTs in men with late stage, metastatic cancer.

 

Rationale: Physical activity after diagnosis is strongly associated with risk of prostate cancer-specific mortality in observational studies. Evidence of the impact of increasing physical activity after prostate cancer diagnosis on biological and clinical outcomes, including recurrence and mortality, from RCTs is needed to change clinical practice.

 

Objectives: The aims for this session are to: 1) present the current evidence supporting an important role of physical activity after prostate cancer diagnosis; 2) compare and contrast remotely monitored exercise interventions among men with prostate cancer; 3) describe safety and feasibility for remotely monitored exercise interventions at different stages of prostate cancer, from low risk disease on active surveillance to castrate-resistant metastatic cancer; and 4) highlight technological solutions (e.g., heart rate monitors, text messages, etc.) to deliver, and measure adherence to, remotely monitored exercise interventions.

 

Summary: During this session, we will have four presentations followed by a moderated discussion. Our overarching theme is the design, feasibility, and adherence to remotely monitored exercise interventions among men with prostate cancer, across the spectrum of disease from low risk to metastatic cancer. Shared features, as well as distinctions, between the interventions being tested will be highlighted and explained.

 

Format: Dr. Van Blarigan will act as Chair and Discussant. She will give an overview of the literature on physical activity and prostate cancer mortality. Dr. Chan will then present on the ASX RCT for men on active surveillance. Dr. Kenfield will present on the Prostate 8-II 4-arm RCT for men receiving radical prostatectomy; and Dr. Hart will present on the pilot CHAMP RCT and the global GAP4 RCT for men with metastatic cancer. Dr. Van Blarigan will highlight areas of similarity and distinctions for remotely delivered exercise interventions across the spectrum of cancer, from low to high-risk disease, and then moderate a discussion between the presenters and the audience on the challenges and lessons they have learned in designing and conducting these trials.

Interaction: In-person audience members will be encouraged to ask questions. As needed, Dr. Van Blarigan will monitor an online symposium throughout the presentations for any submitted questions and read them aloud to the presenters during the discussion section.
